Case management is a critical component of the Families With
Special Needs Program. The primary goal of the outreach case
management services is to ensure that the basic needs of the
family are met, so parents with developmental disabilities or
other special needs can succeed in providing a safe and
nurturing home, and provide advocacy and support to other

rent subsidy and liaison with
landlords
to maintain stable housing
